Enrollment Checklist - New Hires
1. Make your 2012 elections.
- Go to https://portal.adp.com/public/index.htm. Click 'User Login' and enter your User ID and password. If you are unable to login, there are links on the login page for password resets and userid issues. You can also contact the OneBeacon Benefits Service Center at 877.576.0393 for assistance.
- Click the Benefit tab from the ADP home page and select Benefits Enrollment.
- Here you will find your open enrollment event and will be able to make your benefit elections. Upon submitting your elections, you will receive an online confirmation number. Please make note of this number for future reference.
- Review the definition of eligible dependents. You must attest that the dependents you are covering for benefits meet the eligibility definition.
2. Complete your Personal Health Assessment (PHA) if you enroll in either of the medical plans (Choice or Choice PLUS+). You will not be able to complete this step at least 7-10 days after you have made your elections in ADP.
- Visit www.mybluehealthma.com. Click "First-Time Users Register Now". Enter your first name, last name, date of birth, and the last four digits of your social security number (SSN). Then follow the instructions to complete the registration process. Spouses should register using their first name, last name, date of birth and the last 4 digits of the OneBeacon employee's SSN.
- Click on the Access My Health link. Then under Take the Health Assessment section click on the link that states "Complete Assessment".
- Once you have answered all of the questions, make sure to hit submit on the top of the summary page. Once you hit submit, you will see your Personal Health Report.

3. Complete your Biometric Screening if you enroll in the Choice PLUS+ medical plan.
- Follw these instructions to find the closest lab location near you. You must use one of the designated BCBSMA vendor labs. We will not accept your personal annual physical information from a primary physician to fullfill this requirement.
Important Information
- If your PHA is not completed within 60 days, you will be dropped from medical coverage effective your hire date. If your spouse/domestic partner does not complete the PHA, he/she will be dropped from coverage.
- If your biometric screening is not completed within 60 days, you will be moved into the Choice medical plan (with higher biweekly contributions) effective your hire date. If your spouse/domestic partner does not complete the biometric screening, you and your spouse/domestic partner will be moved into the Choice medical plan.
